That is the first sign of fire blight. You'll need to cut off the affected branches back to healthy areas -- when you look at the cut you'll see that the inner layers of the affected branches are brown along the outer bark. MAKE SURE to disinfect your pruning tool with 10% chlorine solution, alcohol, or hydrogen peroxcide AFTER EACH CUT. Dispose the diseased branches in trash. Don't foreget to rake up and dispose of any fallen leaves, though the leaves usually persist on the branch, looking fire scorched, hence the name.
I've been using the 10% milk solution spray in hopes of keeping down the damage, but the first and foremost preventive is to plant resistant varieties of the pear (or apple) tree/root stock.
