HELP!!! I cant ID this plant and its dying.

I really need help finding out what plant this is. I got it from my grandmother and it was beautiful, now its leaves are falling off, it would be easier to figure out what the problem is when I know the plants type!

My grandma said that it blooms like 3 different flowers if that helps

Nope, doesn't help. But if it does bloom and you can post a picture of the flower, that will help a whole lot!
In the meantime can you tell us how you re treating it (watering, fertilizing, etc)?

I actually did alot of research and found.out that it is a bleeding heart vine but thanks to everyone for trying to help. I have repotted the plant and put it outside because I don't think it was getting enough sun.
