How to care for this shade-loving woodland wildflower?

There's a wildflower on my property that is native to this area. I need to know how to take care of it. Things like it's ideal soil type, pH, and how much water it needs would be useful. Its leaves are turning yellow and have holes in them, so I must be doing something wrong.

I can't remember what it is called, but I drew a picture of it so you'll know what I'm talking about:

It's maybe 3 or 4 inches tall. The flowers are white and aren't particularly noticeable because they are dwarfed by the leaves. I think the flowers are bell-shaped.
