Juniper needles turning brown, falling and roots out of pot

Hi,

My husband bought me a juniper a few months ago from a nursery in Central Florida. We put it near our South window, and water it when the soil gets dry. Recently my juniper needles have been turning brown and falling off especially the needles near the trunk. The roots are trying to escape the pot. My juniper is still in his original pot/soil with its rocks (see attached), and the nursery gave us some fertilizer to put inside the pot.

What can I do to make it live? Should I repot it and put a new soil with potting mix and new rocks? Should I fertilize it? What brand(s) of potting mix/rocks/fertilizer do you recommend?
