You can take a finger and move a little soil away from the root to see what you have. One of my radises ended up with no more leaves, now how would I know when a radish is ready to pull out?
Radishes need to make fast growth to make a nice tender root bulb. Insect damage can prevent them from ever making a nice root. Yes, a lot of insects like to feed on the radish leaves. Try some diatomaceous earth.
