Problem with yellow squash

My yellow squash has yellowish leaves on them and some are curling up. I snipped off the really bad ones. When I snipped them off of one of the plants, the plant came right out of the dirt with no problem - it didn't have any roots on it.

I'm wondering if you have Root Knot Nematodes :?: They are more common in warm climates, short winters.

I've had it happen on squash plants and cucumber plants. The only thing I've ever figured out was the soil was to wet around the plants and damping off was occurring at the plant/soil intersection. When I recognized it on a couple of cucumber plants, I piled soil up the stem and created new roots. They are thriving. The pile of soil also prevented additional damping off.
