ID grass + few other problems, please

Hi everyone,

We moved into our home last year & this is our first lawn. If you all don't mind, I have 4 questions please regarding my lawn:
1. Can someone please help me identify what type of grass this is (if pic of individual blades needed, I can get another pic tomorrow)
2. Our lawn has gone to seed all over (you can see in the pic) but we have someone cutting the lawn every week. Should they cut lower? I'm not sure how to remedy this.
3. We have bumps & small mounds all over the entire lawn. What is the best way to get a nice flat lawn out of what we already have here. I'm so afraid to dig out the whole thing & start over. We are working on other projects & I think the cost would be too much to handle right now. We want to at least have a decent lawn for the summer for the kids.
4. How can I create nice borders at the edges of the lawn? Right now the edges just sort of fade out & are quite patchy.
Any help is greatly appreciated
