Revive a Lipstick Plant

I inherited a lipstick plant for someone, and it was very near death. Half the roots were dead, it is very straggly looking. They are a few vine which have healthy looking leaves, most were brittle. I pulled out a few of the roots which were gone beyond repair. Most of the vines just broke off. The vines that are left are around 1.5 feet long.

A couple questions:
A) Should I repot the plant? The soil in its original contain had started to show signs of mold, which I scraped the top inch or two off and kind of freshened it up. I am worried repotting will stress the plant out too much. But on the other hand, I know the pot is full of roots without any viable vines on them, and worry of root rot.
B) Should I trim back the vines that are viable? If I do trim them back I would like to attempt re-rooting the clippings. If they root successfully can I replant them in the existing planter, to make the pot more full? Or will this fullness eventually (after a lot of patience) come back on its own?

The pot looks a little big for the vine. I would use a clean pot especially if you have fungus in that pot. The pot should be scrubbed and bleached before you use it again.

This plant did at one time fill out the pot, but like I said, the majority of it ended up dying and I pulled out everything that was too far gone. Should I repot into a smaller pot?
