Please help, is my tree dying

Hello All,

I'm hoping one of you knowledgeable folk can help me work out whats wrong with my tree.

Over the last 2 weeks I have notices that lots of the leaves are dying and falling off, I may be wrong but I don't remember this happening any other year, the tree is green all year round normally.

I don't know what tree it is but have attached some photos.

Check the trunk for damage. Don't spray any herbicide on the lawn surrounding the tree.

I don't know what kind of tree it is either.

It looks like a shrub to me. I don't recognize it. If you can figure out what kind of shrub or tree it is, that will make it easier to diagnose the problem.
