preparing for garden's first winter?

I did a search but I wasn't sure about the best method so I figured I'd ask!

I have one raised vegetable bed (about 10' by 4'). this year we had tomatoes, cucumbers and squash. we're still getting tomatoes (we're near new york city) but everything else is done.

1. when I finally take down all the plants for the winter, what is the best plan? I was thinking leaves and then straw on top? too much, too little?

2. should I be adding compost on top of the garden before leaves and straw? or leave that for the spring?

3. anything special I should do to prepare my new blueberry plants for winter?
