What's wrong with my rose?

Hi, I'm new here and would like some help with my rose. The leaves are turning yellow and brown and it looks like it's dying. Can anyone tell me what is wrong with it? I don't know what kind of rose it is, I got it at a grocery store in a small pot and planted it in the garden. It did good last year but I transplanted it to another spot and now it's turning brown.

Please post photos (if you have them). The webmaster has given advice on doing so at https://www.helpfulgardener.com/forum/viewtopic.php?t=3724 .

Also provide info on how many hours of sunlight the rose gets, how often/much it is watered, what kind of soil it's in (unamended Florida sand? Florida sand + compost? etc.), how close to the house/driveway/other plants it is, and anything else which may affect its ability to gather nutrients for itself.

The soil is sandy. I added some black cow when I transplanted it. I water it at least once a week - more if it's real dry. It gets mostly full sun. It's a couple of yards from the house. It's in the same area and soil that it was before, I just moved it over some so it's not too close to the other ones. It did good last year.
